What Is a Downturned Smile?
A downturned smile occurs when the corners of the mouth turn downward even at rest, creating a sad, tired, or displeased expression that may not reflect how you feel.
This can result from a combination of aging and muscle activity. As we age, collagen decreases and skin loses elasticity, and overactive depressor anguli oris (DAO) muscles can pull the corners of the mouth downward. Together with the natural pull of gravity, these factors can cause sagging and the appearance of a permanent frown.
Injecting a muscle relaxant, such as botulinum toxin, into the DAO muscles, causes a net upward lift of the corners of the mouth softening the appearance of a downward smile.
