Information Collection & Confidentiality
- We require your contact details (address, telephone and email) in order to secure an appointment.
- New treatments require submission of a completed pre-consultation form, including details of your medical history. This is necessary to inform prescribing and treatment planning.
- All information will be treated as confidential and protected in accordance with the General Data Protection Regulation (GDPR) policy.
- We will not disclose your information to third parties without your written consent, and you will not receive any unsolicited communications from us.
- You may opt out of our mailing list at any time by sending an email to team@revivemeclinic.co.uk with the subject line “Unsubscribe from mailing list.”
Photography
- Before and after images are mandatory for medical records and treatment progression. We will obtain your explicit consent before taking any photographs and/or videos, which will remain confidential and protected under Data Protection Legislation (GDPR). The images serve as an essential assessment tool throughout the treatment process.
- Cosmetic Model Consent: In addition to the above, you have the option to participate as a cosmetic model. Should you choose to consent, you authorise Revive Me Aesthetics Ltd to use your photographs and/or videos for media purposes, including advertising on social media, publications, our website, and other promotional materials.
- You may withdraw this consent at any time post-treatment by contacting Revive Me Aesthetics Clinic.
Appointments
- Appointment reminders are provided as a courtesy via email and/or text. However, please do not rely solely on these reminders, as you are responsible for attending your scheduled appointment. Failure to attend will incur a fee of £50.
- We kindly ask for a minimum of 48 hours’ notice to cancel any appointment. If you cancel with less notice or fail to attend, a £50 fee will be charged.
- If you are feeling unwell, please do not attend the clinic. If you are unsure, contact our team to discuss this further, as many treatments are not suitable when you are ill—particularly if you have cold sores or skin infections in the treatment area.
- Review appointments are offered after treatment with Anti-Wrinkle Injections as a courtesy. Complimentary adjustment doses are not offered after the 2-4 week review period, as determined by the specific treatment received.
Children & Pets
- We do not treat children or young adults under the age of 18 years.
- Please do not bring children to the clinic unless they are old enough to be left unsupervised. They will not be allowed to accompany you into the treatment rooms.
- We do not allow pets into the clinic due to hygiene concerns and the need to maintain a sterile environment for treatments.
- If you do attend the clinic with a child or pet, we reserve the right to cancel or reschedule your appointment.
Payment
- To secure your face-to-face appointment, a redeemable £50 consultation fee is required upfront via bank transfer. A booking confirmation email will be sent once payment is received.
- You will be informed of the full cost of any proposed and agreed treatment plan, including any ongoing maintenance treatments, before any treatment begins.
- Payment is taken, in full at the time of treatment.
- The clinic accepts all major debit and credit cards.
Refund Policy
- The £50 consultation fee is refundable if the appointment is cancelled at least 48 hours in advance.
- The treatment fee includes the delivery of procedure and related services, including consultation (refundable if cancelled 48 hours in advance), provision of information, safe treatment with quality products, and any follow-up care.
- Our experienced professionals are dedicated to providing safe, high-quality care tailored to you. However, as results can vary from person to person, we’re unable to guarantee specific outcomes or offer refunds if expectations are not fully met.
Complaints Policy
If you have a complaint, please feel free to speak with a member of our team or submit your concerns in writing. We aim to acknowledge and respond to all complaints as promptly as possible
Modifications
Revive Me Aesthetics Ltd reserves the right to update or modify these website terms and conditions at any time without prior notice. By continuing to use the website, you agree to be bound by the most recent version of these terms and conditions.
